Insurance Plans

We are contracted with most health plans, and most will cover the initial visit with testing.

To determine your insurance benefits:

• Contact the customer service number listed on your health benefit card
• Ask for your fertility testing benefits and infertility treatment benefits
• You may also wish to inquire about your medication benefit

If you do not have benefits for treatment, you may inquire about adding the coverage during "open enrollment", or utilize our payment plan and discount plans.

We will explain the recommended testing and treatment, and how it will coordinate with your health plan benefits. Please ask questions if you have them!

Cash, check, money-orders and credit cards are accepted. We also offer treatment payment plans by Capital One and our multi-cycle discount plans.

Payment Plans and Discounts

Carrying the full financial burden is not an option for everyone. Since many patients are without health insurance coverage for infertility treatment, we have developed financial options for our patients.

Payment options

• Fees-for-services and insurance filing
  Fee-for-service is our billed fee for service(s). Some contracted health plans may reduce fees based on heath plan benefits and coverages (copays, coinsurance, deductibles, etc., will apply).
   
• Multi-Cycle Discount Package Plans
  FCSA Shared-Risk Program includes the FCSA Refund Plan and the FCSA Package Plan. This Program is administered by FCSA, and designed to help uninsured IVF patients pay for treatment. Subsequent fresh and frozen cycles within enrollment are offered with deep discounts. All terms subject to change.
   
• The FCSA Package Plan
  Our IVF candidates without health insurance have the option to enroll with the FCSA Package Plan. This plan "packages" many services normally associated with IVF and FET cycles. Subsequent cycles are offered to enrollees with deep discounts.  The FCSA Package Plan can be upgraded to include PGD, TESE, egg and/or sperm donor, gestational carrier, etc.
   
• The FCSA Refund Plan
  This plan may offer eligible enrollees money-back if the included cycles within the enrollment terms fails to produce a baby. Not all patients will be eligible for the FCSA Refund Plan.
   
• Active-Duty Military Discount
  With proof of status, our active-duty military patients qualify for discounts off out-of-pocket services billed by our center.
   
• Two Payment Plan Options Administered by Capital One and CareCredit
  Paying for treatment just got easier! We offer two qualified payment plans admistered by Capital One and CareCredit. Online enrollment available. The payment plans can be used for purchasing medications, screening, tubal reversal surgery, fertility testing and treatment, and package plans. APR as low as 1.9%. Payments can be combined with any other financial service, as well.

The programs herein are not health insurance and do not provide health insurance coverage, nor do they guarantee pregnancy or live birth. These plans exclude fees for medications, screening, complications, OB care and other services.
